Student Health Services/Nurse's Office

The National Association of School Nurses defines school nursing as:
A specialized practice of professional nursing that advances the well being, academic success, and life-long achievement of students. 

To that end, school nurses facilitate positive student responses to normal development; promote health and safety; intervene with actual and potential health problems; provide case management services; and actively collaborate
with others to build student and family capacity for adaptation, self management, self advocacy, and learning.

The nurse's office is open from 8:00 am until 3:00 pm Monday thru Friday. Students and Parents are encouraged to discuss any health problems with School Nurse Kristine Gay RN, by calling 518-576-4555.

Below are links to the Permission form for School Physicals, the actual physical form that would need to be filled out by one's own MD, the HIPPA Form, the health history form and a Medication Permission form should your child start a new medicine that needs to be taken during school hours.
